consult questionaire for the area of law in which you require assistance. This is the same form
that you must complete when you appear for an in-person consultation, so completing it in advance saves you time and provides
me with more accurate information. Faxing it to me in advance allows me to become familiar with your situation before the
consultation, which means more time can be spent during your consultation explaining your options. Finally, a telephone consultation
may save you an unnecessary trip to a lawyer's office, saving you time and the expense of a babysitter or transportation. Unfortunately,
due to the new bankruptcy legislation, I can no longer discuss bankruptcy matters by phone unless you complete, sign,
and return the consult forms. Thank you.
